Payment information

Currency

All prices stated on the current website are denominated and charged in New Zealand Dollars (NZD) for New Zealand residents. International customers will be charged in the currency noted on the site prior to completing the purchase. If currency conversion is required, your bank may charge you a fee for this. Please refer to your bank terms and conditions for more information. When purchasing from New Zealand, the advertised price and the price of your order is inclusive of GST.

How do we protect cardholder data?

All transactions on the website use best-practice encryption and security including SSL/TLS. We do not store cardholder data for longer than is necessary to process the payment through our payment gateway and these details are never stored in our database at any time. Your Lifestyle Medics staff, contractors and developers do not have access at any time to card data entered into the website.

Occasionally, at our discretion, we may include or offer third party products or services on our website. These third party sites have separate and independent privacy policies. We therefore have no responsibility or liability for the content and activities of these linked sites. Nonetheless, we seek to protect the integrity of our site and welcome any feedback about these sites.

Your Lifestyle Medics monitors and analyses website traffic to improve our services, this can be used to keep track of User behaviour. Google Analytics is a web analysis service provided by Google Inc. (“Google”). Google utilises the data collected to track and examine the use of this website, to prepare reports on its activities and share them with other Google services. Information for your lifestyle consultation

Please let us know if you:

➢ Are pregnant, trying to get pregnant or become pregnant during treatment

➢ Have any allergies

➢ Have any serious medical conditions or have any physical or mental health conditions

that may affect our assessment and treatment of you.

➢ Have any changes in your medical condition during your time of your treatment.

➢ Have any concerns about your proposed treatment plan or problems resulting from

your treatment plan.

➢ Have any particular cultural needs or requirements that we should be aware of.

Lifestyle consultation fees

➢ Payment is required BEFORE your appointment, either by credit card (via online

booking system), or internet banking.

➢ Any appointment cancellations must be made at least 24h in advance, any

cancellations made within 24h will incur a fee of 50% of the appointment cost

➢If you do not attend an appointment a 50% ‘did not attend’ fee will be charged.

➢Appointment costs:

★ First appointment $285 (1 hour)

★ Follow up appointment $140 (1/2 hour)

Payment can be made by credit card/paypal at time of booking online, or via Internet banking

CONSENT FOR TREATMENT

I consent to treatment, and understand that a full verbal explanation of my treatment plan will be

given at time of consultation, and a summary will be emailed to me within 24 hours of my appointment.

I understand I have the right to seek a second opinion at any time. I agree to keep Your Lifestyle

Medics informed of any changes in my medical condition or circumstance that could affect my care.

CONSENT FOR TREATMENT

In accordance with the Privacy Act, all information recorded in your health records will be kept strictly

confidential. All personnel in this practice are required by law to maintain strict patient confidentiality.

Records are stored in a secure patient management system on our computer, or in hard copy.

Under the Privacy Act you have the right of access to, and correction of, your personal information

held by this practice. No information will be given to a third party without your consent unless required

by law.

I understand that if this data is collected for audit purposes, no information will be collected that

would allow me to be personally identified.

RIGHT TO SUPPORT AND ADVOCACY CONSENT

i understand that I am entitled to have a support person(s) with me when undergoing assessment or

treatment, except where safety may be compromised or other patients rights may be unreasonably

infringed on. I also understand I am entitled to advocacy services, and can access these through

Health and Disability Service (Ph 0800 11 22 33)

CONSENT FOR PAYMENT

i understand that:

If I fail to attend my appointment, or cancel withou 24 hours notice, I will be charged 50% of the

appoinment fee

If I fail to pay prior to/on the day of my appointment I will be charged an administration fee unless

prior arrangement has been made

If debt recovery service is required to recover my debt, I will be held liable for any debt recover fees,

PLUS an administration fee of $50

Information and statements regarding dietary supplements have not been evaluated by the Food and Drug Administration unless specifically stated.

Your Lifestyle Medics virtual/online events

We do not offer refunds or credits for our virtual events if you are unable to attend a planned event that occurs at the time advertised. You are welcome to transfer your ticket to someone else if you are unable to attend – please advise us of this so we can update our records.

You are not permitted to record any online events– whether for distribution or personal use. We provide recordings for some online events for those who have purchased a course or place to participate in these. To record any online events without permission constitutes a breach of Your Lifestyle Medics intellectual property and copyright and appropriate action may be taken in this regard.

If a virtual event cannot be conducted at the planned date and time, due to circumstances beyond our control, a reasonable alternative will be provided which may include, but is not limited to, a new date or time.  Attendees will be notified as early as possible in the unlikely event that this occurs.

Force Majeure

Your Lifestyle Medics Ltd will not be liable for any failure to comply with our obligations where the failure is caused by something outside our reasonable control. Such circumstances shall include, but not be limited to, weather conditions, fire, flood, hurricane, strike, quarantine, industrial dispute, war, hostilities, political unrest, riots, civil commotion, inevitable accidents, supervening legislation or any other circumstances amounting to force majeure.

Comments, feedback, testimonials

If you provide us with comments, feedback or testimonials, whether written, verbal or otherwise, you consent to us using this information and in our marketing resources. Please note we will not identify you by name unless you have given us permission to do so. Courses and membership

The contents of online courses and membership are for informational purposes only and are not a substitute for professional medical advice, diagnosis, or treatment. Refund policy

If you purchase one of our courses or resources and are unhappy with your purchase, we offer a full refund within 7 calendar days from time of purchase, provided you contact us directly via the contact details provided on our website within these 7 days. After 7 days from date of purchase no refund will be provided unless our product is faulty under the consumer guarantees act.

Please choose online courses carefully before completing your order, as no refunds, credits or transfers are available for online course purchases and enrolments after 7 days from date of purchase.

Reboot your health online course

Enrolment in Reboot Your Health provides 6 months of access to the 6-week course, commencing from the date course was purchased. After this time access automatically expires. 

All written, audio, and video resources in this course are protected by copyright, and are the exclusive intellectual property of Your Lifestyle Medics. Written resources from the course may be downloaded and must not be used for any purpose other than private non-commercial use. An internet connection is required to access this online course.
